Telos is not a HIPAA covered entity
Telos MD LLC is a marketing and advertising company. It does not provide health care, bill insurance, or operate a health plan, so it is not a covered entity under HIPAA, and it does not receive protected health information. This page exists because being honest about that boundary matters: it tells you exactly where HIPAA protection begins in your journey.
What the quiz collects
The quiz on telos.md collects marketing and routing fields: goals, general interests, and contact details. It is not clinical intake, it does not create a medical record, and your answers are not protected health information. They are handled under our Privacy policy.
Where HIPAA protection applies
Clinical information you share with the affiliated medical group, the platform, and its pharmacies and labs, including intake forms, consultations, lab results, and prescriptions, is protected under their HIPAA obligations. Each of those entities maintains its own notice of privacy practices, which you will see during checkout and intake on shop.telos.md.
Where to direct HIPAA requests
Requests to access, amend, or restrict your medical records, or to receive an accounting of disclosures, go to the platform and the provider you completed intake with, not to Telos. You can reach them through the support and patient portal links you received at checkout on shop.telos.md.
